Reiki and Sound Healing
I understand that Reiki and Sound Healing are integrative, non-invasive techniques used for stress reduction and relaxation.  I understand that Reiki and Sound Healing practitioners do not diagnose conditions nor do they prescribe or perform medical treatment, prescribe substances, nor interfere with the treatment of a licensed medical professional.  I understand that Reiki and Sound Healing are complementary treatments and do not take the place of medical care.  It is recommended that I see a licensed physician or licensed health care professional for any physical or psychological ailment I may have.  I understand that Reiki and Sound Healing can complement any medical or psychological care I may be receiving. 

Massage Therapy
I understand that the massage I receive is provided for the basic purpose of relaxation and relief of muscular tension.  If I experience any pain or discomfort during this session, I will immediately inform the therapist so that the pressure and/or strokes may be adjusted to my level of comfort. I further understand that massage should not be construed as a substitute for medical examination, diagnosis, or treatment and that I should see a physician, chiropractor or other qualified medical specialist for any mental or physical ailment that I am aware of.  I understand that massage therapists are not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat any physical or mental illness, and that nothing said in the course of the session given should be construed as such.  Because massage should not be performed under certain medical conditions, I affirm that I have stated all my known medical conditions, and answered all questions honestly.  I agree to keep the therapist updated as to any changes in my medical profile and understand that there shall be no liability on the therapist’s part should I fail to do so.

Appointment Booking Fee:

I agree to a $25 booking fee to reserve my appointment.  This fee is applied to the balance due after my completed healing session.  This fee is fully refundable only if I am cancelling or rescheduling an appointment 24 hours in advance.  This fee is non-refundable if I am cancelling or rescheduling an appointment within 24 hours of the original scheduled time.

Cancellation Policy:

Should you need to cancel or reschedule, please give notice at least 24 hours in advance. 
Any cancellations with less than 24 hours of notice are subject to a cancelation fee amounting to the cost of the scheduled session. ​
